Devoted Health sells five plans in Ohio: Choice, Choice Extra, Core, Giveback and Premium. Choice, Choice Extra, Core and Giveback are all free Medicare Advantage plans, although you will still pay the standard $185 each month for Medicare Part B.

The Devoted Health Choice or Choice Extra plans are best if you want the flexibility to choose your doctors. That's because they're both PPO plans. PPOs let you use any doctor and still have some coverage, but you'll pay more to see doctors that aren't in the network. And you can see a specialist without a referral. You have to pay the first $590 for your prescriptions, though. And you could pay up to $5,300 in medical bills each year before the plan begins to cover all your health care costs.

Devoted Health's Medicare Advantage plans include several extra perks. Your plan might have dental, vision and hearing coverage, a free gym membership and a rewards program.

If you tend to have high medical costs, the Devoted Health Premium plan is the best option. It costs $6 per month, but it pays for more of your medical bills than Devoted Health's free plans. The most you might pay in a year for medical care is $4,500. After you hit that amount, the plan pays for all of your covered care. Keep in mind that Premium is an HMO, so you have to stay within the doctor network to have coverage. You also have to get specialist visits approved by your primary care doctor.

Extra perks include rewards for healthy activities, money for over-the-counter health products like bandages and cold medicine, and $0 virtual care. Your plan might also let you get some lab tests, like blood work, for free. Most plans include dental, hearing and vision coverage. Perks are different depending on the plan you buy, so be sure to review your plan documents.

AARP/UnitedHealthcare assigns numbers, instead of names, to its Medicare Advantage plans in Ohio. Some plans, like 0004, have four numbers. Others are a combination of the state abbreviation and a number, like the OH-9 plan.
